IDENTIFIKASI DRUG RELATED PROBLEMS TERAPI DIABETES MELITUS TIPE II DI KLINIK WILAYAH KABUPATEN BANDUNG BARAT

ABSTRAK
Penyebab kematian ketiga tertinggi di Indonesia adalah Diabetes melitus (DM) yang merupakan penyakit menahun serta memiliki tingkat komplikasi sangat tinggi. Tatalaksana DM adalah tindakan terapi obat. Oleh karena itu perlu hati hati untuk menghindari masalah terkait obat. Tujuan penelitian ini untuk mengidentifikasi kejadian Drugs Related Problems (DRPs) pada terapi pasien DM tipe 2 di salah satu klinik wilayah Kabupaten Bandung Barat. Pengambilan sampel menggunakan metode incidental sampling dari 254 rekam medik dan catatan perkembangan pasien secara retrospektif dan prospektif observasi kadar gula. Identifikasi DRPs menggunakan Pharmaceutical Care Network Europe (PCNE) drug-related problem classification version 9.00. Uji statistik chi square dari data yang terkumpul diolah dan dianalisis secara deskriptif. Hasil penelitian terdapat laki-laki 42,52% dan perempuan 57,48% dengan kriteria usia >60 tahun 55.91% dan <60 tahun 44,09%. Penyakit penyerta terbanyak adalah pasien gangguan kardiovaskular sebesar 29,31%. Penggunaan obat antihiperglikemia oral paling banyak yaitu 68,1% metformin tunggal. Kasus kejadian DRPs paling banyak 202 (84,5%) pada kriteria pemilihan obat tidak tepat, 17 (7,1%) dosis pemberian dan 20 (8,4%) untuk masalah terkait pasien dari 239 kejadian DRPs yang terjadi pada pasien DM tipe 2. Keberhasilan terapi dapat dilihat dari rata-rata kasus DRPs 94% dimana penurunan kadar gula darah puasa 17,3%, serta penurunan kadar HbA1c 18,8%. Hubungan keberhasilan terapi berdasarkan chi square dengan nilai P Value <0,05 menunjukkan adanya hubungan yang signifikan antara masalah terkait obat dengan tingkat keberhasilan terapi dimana semakin besar persentase kejadian DRPs, semakin kecil nilai delta GDP dan HbA1C.
